Credentialing Requests

Credentialing Request Procedures

All credentialing requests will be processed in the order that they are received in our office. Please allow up to 14 business days to handle your request. To process your request, we need to have a written request sent by mail or faxed to us that includes the following information:

  1. Either the provided form from the credentialing company or a written request which includes the name and address of the third party that you are requesting the credentialing verification information to be sent. The credentialing companies generally require that the verification be sent directly to them rather than returned to the alumnus.

  2. Your name, address, phone number, social security number and, if possible, your matriculation number.

  3. We must have a signed signature in the letter, as we do often compare signatures. Please do not use a signature stamp. If the request if being sent to us directly by the third party, generally an insurance provider, a signed release from the doctor must be included before we can release any information to that third party.

  4. There is a $15.00 fee per credentialing request, which may include a certification letter and/or transcript. This fee may be a check payment or money order made payable to Palmer College. We also accept MasterCard, Visa and Discover. Please include the card number, expiration date, and the name on the card. All fees are due before the credentialing request can be processed.

  5. If you would like to have the credentialing request mailed to the third party overnight, please indicate so in your letter and include an additional $15.00 to cover the overnight fees if to be mailed within the U.S.  For overnight requests being sent outside of the U.S., please the number below for the rate fee.

Or, you may choose to fax the request to us at (563)884-5864. All faxed requests must have credit card payment information included in the request. Faxed requests are handled in the same manner as mailed requests.
